Where: Blue Springs Baptist Assembly ~ 5000 East U.S. Highway 90 ~ Marianna, Fl 32446
When: Monday, August 11th through Saturday, August 16th
Who: Camp is mandatory for all football players 9th through 12th

The Quarterback Club Questionnaire is an important form to be completed by a parent for each player. The form captures personal information for communication purposes and will provide an opportunity for you to select areas of interest and involvement in the Eagle Football program. The information collected is for the sole use of the Quarterback club. Please complete one form per player, as soon as possible. Completed forms are to be returned to Barbara Adkins by e-mail (cityoflite@aol.com) or by fax (850-942-7368). If you opt to fax the form, please include both sides; or, you may send the completed form to: ATTN: QB Club Questionnaire, 1400 Village Square Blvd., Suite 3 Box 197, Tallahassee, FL 32312.

Come out and support the Varsity and Junior Varsity Eagles as they scrimmage on Eagle
Field. Game admission is free and the BBQ dinner is $7.00 per plate.
